How can you photoshop images? Is there a program that lets you create images that arent real?

For example, this image here, everything except probably the background is COMpletELY fake.

Is this done in photoshop or some other program? Is it easy?

Any program that allows you to edit a picture is, to one degree or another, creating something that isn't real. That said, I know what you mean, and yes, there are any number of them.

"Photoshop" has become a term, something like "Kleenex", used to denote a whole host of similar products.

Recognize that while images are often made up of dots (pixels) -- called bitmap images -- they can also be made up of geometric elements -- and called vector images. Digital cameras take bitmaps, while programs like CorelDraw create vector images. The advantage of vector is that they can be scaled very well and allow some really interesting effects not readily achievable with bitmaps (e.g. 3D fonts, technical drawings, Logos). Bitmaps on the other hand don't scale as well but can be edited extensively -- from basic changes like brightness, contrast, to more advanced effects like colour replacement, cloning and erasing or specific portions (down to the pixel level of detail) and overlays of other elements or even morphing (graduated change from one image/shape to a different one (e.g. a human face into a Lion)).
